Hm, I haven't looked at your intake form Alan as I've been visiting my sister for a couple days and thier computer doesn't have all the programs that I have, and even checking on the activity here at MassagePlanetL is a little harder on their computer. I'd love a laptop.... *sigh*

But I did want to point out that if 30 people had opened your PDF file, and you are concerned that nobody else has shared theirs... it could be that none of the members who have checked out your intake form have theirs available online to share in such a manner. I don't have mine available online... I don't even have a website... :?

That being said, I think for most of us intake forms are sort of a "personal" thing. Things which one therapist may feel are vital another may not wish to have on the form at all. It just depends on the therapist and their practice.

Have you tried using the search function on the board here to find similar threads about health intake forms? It's just a thought!

I do not have my intake form online but I would be happy to email it to you in a word document.

It would be interesting to have a thread exclusively for links to intake forms we use. I formed mine after viewing other therapists websites/intake forms. But I do not keep mine online because I just have clients fill it out when they come in.

I have not viewed or downloaded your forms. I would guess that most who have simply cannot post a link.

The forms that I use were purchased as part of either a marketing materials package or an insurance billing package. These forms are copyrighted material and are a business for the massage therapists who did the work to put them together for sale. I would not want to harm these women by making those forms available through this forum.

Alan,
Mine is so similar to yours that you'd think we came up with it together! ha I don't have an acrobat version of it, but could pull one together & send. I changed & honed it over the years, weeding out some things, adding others. I think I first based it on one that was circulating a BPE class in school and some points I thought were valuable to ask that were on the intake form at the first couple of places I ever worked.

BTW, don't be discouraged if others don't reply as often as they read through other postings. Its just the nature of things on a message board. Today, for instance, I have participated madly compared to other times when I am just looking for a great read or the benefit of others' views.

Um, I saw this thread but didn't look at the forms and didn't post because I thought it was about questions regarding what to include on forms. I don't need to see others' and am not able to share my own, since it's not in a format I can upload.

Like Sage, my first one was cobbled from class and what I thought I needed where I was working (they didn't use them) and it's been weeded out and added to over the years to make it as brief and easy to read and use as possible, yet still get the input I need.

Intake forms are like resumes in that they should NOT exceed one page (exception for insurance billing/workmans comp/PIP). Therapists with six pages... :roll: I don't want to feel as if I'm at a physician's office. Just that impression starts making me nervous and takes away from the experience.

I do not have my intake form online either. Mine is one page, and I still get an occational complaint when I give it to a new client to fill out.

I started with the one ABMP provides, but I had many complaints that the print was too small. So, I had my mom redo it in larger print and make a few changes to it as well. I included a paragraph about missed appointments, added a few more questions, and added a few more blanks for such things as email address and cell phone number.

I don't have mine online yet (working on that), but I did look at yours (didn't copy it, just looked at it). Reading yours reminded me that it would be good to put a "who referred you" type of question on mine.

I agree with the others, don't get too bothered by people reading but not yet responding. I know if I don't have a lot of time, I might quickly look through, and plan to respond later.

I have the worst memory ever. I have the intake so I can remember how to contact them, remember which side of the body they have their booboos, and remember how they got to my practice, (mainly because I have a fair amount of people self-refer from my website.)

Then I also keep brief notes on every session, because I can't remember who got what treatment by the time they show up again a couple weeks later. I wish I had that kind of great memory, but I think I'm too easily distracted by shiny objects and flash animati... oooooh! look at the pretty fishy!....

I have a one page intake form which I go over with my client, and make my own notes. That way I'm not asking them to do the chore of filling it out, instead I am chatting with them about their health history. I also find that I get more information that way as their verbal answers will sometimes prompt additional questions from me, leading to important information that would otherwise be missed. Yes, it takes more time, but I schedule my appointments to take that into account.

A few times I've been in a time crunch (clients are here on a cruiseship, I have 6 hours to give 5 one-hour massages). In those cases I email the intake form to the clients to fill out in advance. While I do look them over before the massage and ask additional questions, invariably I find that the client remembers one more thing while on the table. For instance I'll say, "I see you have a surgery scar on your right knee," and they respond, "Oh yeah, I forgot, I had surgery on that knee about a year ago." This happens even though there's a question about surgeries on the form. Granted this can happen when I go over the form with them, but it is definitely more frequent when they fill it out.

In addition, by chatting with them prior to the massage, it helps to put the client at ease. It's part of the relaxation process. It also gives them some time to enjoy my beautiful massage space instead of staring at a form prior to being face down in a face cradle.

On the backside of the intake form is the consent form, which includes something to the effect that all the information given is true to the best of their knowledge and that they presently have no condition that would make the release anything other than knowing and voluntary.

Alan, I noticed that the sidebar on your website is alphabetical, which puts "Desserts" before the information about your services. Although I'm a great proponent of "Eat Dessert First!", you might want to rearrange. Just my two cents...
