I thought “hard” was code for HJ. And “medium” was not? I’ve tried both but nothing is consistent.

There's no code.
Hard massage is exactly what it suggests. More pressure.
I never take a hard massage unless I know the MPA really knows what she's doing. Otherwise there's too much risk of coming out with a sore back or neck.
If you want to play with that question, just tell your MPA whatever type of massage it is that you want, as long as she makes you hard.

Hard, medium, soft refers to the amount of pressure you prefer during the massage. FS shops don't ask this since there is none or little massage

HE/He = happy ending meaning you'll get minimum a hand job
in some places leaving your underwear on means You don't want a HE
In some places on the flip they ask you: "Is there anywhere else you want me to massage?"
it varies by shop or by attendant there is no standard, you simply have to play it by ear

for sure: on the flip put your hand on their ass, if they let you they will follow up with offerings
if they don't do extras they will freak out but don't worry you are not gonna get a slap LOL
