Treatment Intake Forms

I know that many MTs use intake forms to get information about clients before they begin a massage. Being a reiki practitioner i was feeling that I want to do the same thing for my Reiki clients. What else besides name, and pertenant information should I ask? I have seen on intake forms about abuse prior to the client coming in (sexual, physical, etc) as well as just what they expect to get out of the treatment. I plan on including those things but is there anything else I should include? Thank you so much.

When I do a Reiki session on a new client I use the same form as I would for a new massage client. Areas of concern, past illnessess, yada, yada, yada. That way I know what is going on with a few questions from me to clarify things. It helps me to know what to look for and what I could expect. Beyond that you may ask if they have had energy work before and what the outcome was, or you could ask them personally. It sounds like you will have all your important information, I think you'll do fine!

I don't use intake forms. Since Reiki does no harm, there really isn't much you have to look out for...only thing I could think of is if they were under radiation treatment. I ask a few of my standard questions from massage (how is your energy today? Are you currently under radiation treatment? etc) talk to them about their ailment and start treatment.

I have seen it on massage intake forms because sometimes if someone is abused physically they may not feel comfortable being touched at all but may not know it entirely so I would ask, if they checked off yes if they are comfortable with a reiki treatment as it does involve minor touching. I would also say I do not have to touch you if you do not feel comfortable. I guess it is something I have seen on other intake forms and thats why I was asking. Thank you for your thoughts though

Hmm...I wouldn't ask specifically on any form if there had been any kind of abuse...most people aren't too keen on talking to just anyone about that. The forms I am familiar with ask if they are currently seeing a mental health professional (check yes or no) with the option of explaining why if yes. I think if people aren't comfortable with being touched they won't be coming to you in the first place, and if they are going to have an emotional release there isn't much you can do to stop it.

makes sense to me Ahimsa!!! Thank you for your thoughts. I never thought about it entirely that way especially about the emotional release but thank you very much.....I think I am just going to do a small one with just one question besides name and stuff and that will be simply what do they expect during a treatment. Thank you again.

I use intake forms to keep track of the client's original purpose for Reiki treatments, as well as their preferences in music, time of appointment etc. One client doesn't like hands near her throat, so when I review her file before her appointment, I am reminded of this fact and keep well away from that area. Without the form review, I could easily overlook this fact.

I never get into their personal lives and if they volunteer, I don't form an opinion. Sometimes they just need to talk to someone who'll just listen.

There are some good organizations, like the International Association of Reiki Professionals who have forms you can download and print off. I had to reword a few questions to be more applicable to my situation, but the download is a good guideline. You must become a member to do this, but there are so many benefits to being a member, including liability insurance.

I don't get into personal lives, but I defo have them sign an intake form. It is different than my massage as it very thoroughly states I am not a physician and I do not treat mental illness, etc. Sometimes you can get people who have real issues and it's best to protect yourself. reiki does no harm, but let's face it, we are living in a non perfect world.

Isn't that the truth!? I get it in my Tai Chi classes too. Sometimes people confide in me with the strangest things. It has helped me develop some good, lasting friendships though.
